Zirconium or Porcelain?

Both look white, both appear strong. But the difference between them emerges over a decade. A guide to making the right choice.

/ Zirconium For Strength

Zirconium (zirconium dioxide) crowns: an exceptionally strong, metal-free ceramic that reflects light well. Its higher fracture resistance is why it is chosen for posterior teeth, in regions exposed to high chewing forces. Its colour remains stable and is unaffected by chromogenic drinks such as tea and coffee. One limitation: monolithic zirconium can occasionally appear overly opaque on anterior teeth, making it harder to replicate the translucency of natural tooth structure.

/ E-max For Natural Appearance

E-max (lithium disilicate ceramic) crowns: a high light-transmission option. Thanks to its thin, light-transmitting structure, it gives a result close to the appearance of a natural tooth on anterior restorations. However, its resistance to chewing forces is lower than zirconium, and it is not recommended alone for posterior restorations. It is the material chosen for Hollywood Smile cases and laminate veneers.

/ Traditional Metal-Ceramic Crowns

Traditional metal-ceramic (PFM) crowns: the standard choice of ten to fifteen years ago. Porcelain is fused to a metal substructure. The drawback: the metal can become visible over time, and a grey line may appear at the gum margin. Which crown is best for my back teeth?+

Fracture resistance is what governs the posterior region, and zirconia leads on that measure. But "E-max cannot go in the back" is not accurate: across 627 monolithic lithium disilicate crowns placed on posterior teeth in 335 patients, survival was 97.93% at a mean of 48 months, with only nine crowns replaced because of fracture (PubMed 28740971). The choice follows the remaining tooth structure, the occlusion and whether bruxism is present.

Which material looks most natural on front teeth?+

On measured translucency E-max leads: of three materials compared with a spectrophotometer, the highest translucency parameter belonged to E-max CAD and the lowest to conventional high-strength zirconia (Katana HT), with next-generation cubic zirconia (5Y-PZ) sitting between them (PubMed 37929525). Clinically the gap is narrower than that suggests: in a controlled trial following 60 veneers for three years, no significant difference appeared between the two materials on any esthetic, functional or biological criterion (p>0.05), though E-max scored better specifically on esthetics and tooth translucency (PubMed 37787028). So E-max remains the anterior reference, while newer translucent zirconia is closing the distance.

Do these crowns stain from tea and coffee?+

Zirconia resists staining but is not immune to it, and the numbers say so. After 28 days immersed in tea, colour change stayed at ΔE 3.0 ± 0.33 for extra-translucent multilayer zirconia while polymer-infiltrated ceramic reached ΔE 8.06 ± 1.04 under the same conditions, roughly two and a half times as much (PubMed 39364838). Zirconia's figure sits just under the perceptibility threshold (ΔE≈3.3), so it is not expected to be visible to the eye. Acidic drinks produced no perceptible change over a week of immersion, while coffee affected the colour stability of every zirconia sample, varying between products (PubMed 29302281). So the crown resists staining rather than being immune to it.
